It is relatively simple to do, Xtream-Codes generally requires 3 crucial parameters to work:

1- Name: any name can do it, this parameter is just used to identify a subscription in case you have several.
2- Username*
3- Password*
4- Link*

An M3U link usually looks like this:

http://provider.com:8000/get.php?username=123456&password=xyz&type=m3u&output=mpegts

To convert it to Xtream Codes, and you have to start viewing it in 3 parts:

1- URL part = http://provider.com:8000 which we will paste on URL
2- Username is equivalent to username which is = 123456
3- Password is equivalent to password = xyz

And there you have it, all you have to do is insert the data and the final result should be as follows
